🧾 About the Role
The Clinical Health Network for Transformation (CHN) is hiring a Patient Navigator to support abortion access and equity. In this full-time, fully remote role, you’ll provide trauma-informed, logistical, and compassionate case management to patients navigating abortion care. This includes helping with travel arrangements, legal referrals, and access to funds—while collaborating across affiliates to improve the patient experience system-wide.

📋 What You’ll Own
• Coordinate appointments, schedules, and follow-ups for abortion patients
• Support a diverse caseload with resource referrals, travel logistics, and financial assistance
• Liaise between patients, support networks, healthcare staff, and external orgs
• Monitor incoming requests across phone, email, and chat channels
• Book transportation, lodging, and provide real-time travel support
• Maintain accurate records, document patient touchpoints, and support follow-through
• Promote and improve workflows across affiliates for consistent, quality care
• Ensure continuity of care before and after abortion procedures
• Model equity-centered, patient-first communication at all times
🎯 Must-Have Traits
• Bachelor’s degree in Public Health, Social Work, or 3+ years relevant experience
• Deep understanding of reproductive health and abortion care landscape
• Strong written, verbal, and interpersonal communication skills
• Organized, detail-oriented, and able to manage multiple priorities
• Experienced in trauma-informed care, case management, or client advocacy
• Tech-savvy: Microsoft Office, EHR/EMR systems (EPIC preferred), Google Workspace
• Comfortable working flexible hours including nights/weekends as needed
• Committed to racial equity, patient autonomy, and inclusive care

• Knowledge of U.S. geography, time zones, and travel logistics
• Prior experience working in social work, patient advocacy, or access-focused nonprofits
