Care Coordinator (Bilingual)

As a Care Coordinator, you will play a key role in assisting patients to access essential medical services, secure durable medical equipment, and connect individuals to vital community resources. Your critical thinking, problem solving abilities, and unwavering tenacity in seeking resources will be key in ensuring our patients receive the best possible care. You will collaborate with patients, dialysis clinic staff, nurse case managers, insurance providers, physicians, community resources, and vendors to improve health outcomes. As a Care Coordinator, you will demonstrate adaptability, thoughtful decision making, compassion, and resourcefulness.

The work you will do:

  • Locate providers that accept patient’s insurance and new patients
  • Schedule appointments with providers and efficiently coordinate patient care
  • Secure durable medical equipment and home health services for patients in need
  • Connect patients to vital community resources to enhance their overall well-being, including housing and shelter placement
  • Overcome barriers by thinking creatively and finding innovative solutions
  • Opportunities to assist with various projects

The skills and qualifications you need:

  • 2+ years of related experience
  • Must be bilingual – verbal and written Spanish proficiency required
  • Basic knowledge of insurance benefits and eligibility criteria
  • Highly self-motivated, dependable, excellent time management, and ability to work independently
  • Strong verbal and written communication skills
  • Ability to maintain effective interpersonal relationships at various levels of the organization
  • Dialysis or case management experience is highly valued
  • Basic computer skills. Familiarity with Microsoft Office applications preferred
  • Have a secure private office area that allows for protection of PHI
