Schedule

  • Remote, full-time position
  • Pay range: $20–$22 per hour

What You’ll Do

⦁ Maintain client files and all related paperwork, ensuring accurate, up-to-date documentation of services, conditions, and care plans

⦁ Protect client and agency information by following HIPAA guidelines and all privacy requirements

⦁ Assign homecare aides based on the client’s overall needs and Plan of Care, balancing quality of care and staffing

⦁ Provide coaching and feedback to caregivers to support high-quality client care, escalating ongoing performance concerns to the caregiver management team

⦁ Communicate proactively with clients, homecare aides, referral sources, and management about schedules, changes, and service updates

⦁ Prioritize client care and, where permitted, conduct in-home visits to stay connected, assess needs, and communicate changes to appropriate parties

⦁ Prepare and submit routine departmental reports and maintain accurate records of client condition and services provided

⦁ Track client satisfaction surveys and in-home visit notes, addressing areas of concern and strengthening relationships

⦁ Handle client complaints and critical incidents as a mandatory reporter, documenting and reporting issues for follow-up and resolution

⦁ Provide education and coaching to clients and caregivers on changes to the Plan of Care

⦁ Support compliance with local, state, and federal regulations as well as company policies and procedures

What You Need

⦁ At least 18 years of age

⦁ High school diploma or GED required; bachelor’s degree preferred

⦁ Medicaid, waiver, or home healthcare experience preferred

⦁ Excellent organizational skills with the ability to multitask and manage multiple responsibilities at once

⦁ Strong problem-solving skills and the ability to handle conflict in a professional, solutions-focused way

⦁ Compassionate, responsible, and positive attitude when working with clients, caregivers, and partners

⦁ Basic computer literacy and typing skills

⦁ Ability to move or position up to 50 pounds and travel within offices and/or client homes, including stairs (with reasonable accommodations as needed)
