What You’ll Do
- Provide timely, professional treatment updates to referral sources (hospitals, schools, practices, agencies).
- Build comprehensive, clinically appropriate discharge plans and coordinate aftercare placement.
- Use a nationwide provider database to identify resources; send referrals via preferred channels and confirm placement.
- Document contacts, maintain accurate progress notes, and keep internal teams current on case status.
- Partner cross-functionally (clinical, admissions, outreach, partnerships) to remove barriers and meet service levels.
- Track and meet performance targets for quality, timeliness, and placement success.
What You Need
- Bachelor’s degree in health sciences, communications, psychology, social work, or related field.
- 2+ years in healthcare (case management, discharge planning, referral relations, admissions, or outreach).
- Clear, empathetic communicator with strong relationship-building and listening skills.
- Metrics-driven, highly organized, and detail-obsessed; excels with deadlines and changing priorities.
- Proficient with Salesforce and Google Suite/MS Office.
