Schedule & Compensation
- Remote, U.S.-based
- Independent contractor (1099) role
- Full-time: $6,800 – $7,400 per month
- Part-time (20+ hours/week): $3,600 – $4,400 per month
What You’ll Do
- Build strong, trusting relationships with Medicare patients using empathy and action-oriented guidance.
- Identify patient needs and create holistic care plans addressing social determinants of health (SDOH) such as food access, transportation, and home support.
- Navigate patients through complex healthcare challenges to achieve better outcomes.
- Learn and utilize Solace systems, technology, and partner resources while contributing your expertise.
- Help design and refine future healthcare advocacy systems through feedback and collaboration.
What You Need
- 3+ years’ experience in care management, patient advocacy, or healthcare navigation.
- Proven ability to work with diverse patient populations and address SDOH.
- Strong clinical knowledge, organizational skills, and ability to balance multiple cases.
- High technical fluency and comfort with learning new systems.
- A compassionate, action-driven approach with fearless feedback and advocacy.
- Must be U.S.-based.
