If you’re an LPN who likes fast-paced phone-based care, problem-solving, and helping new patients get set up the right way, this role is a strong fit. You’ll complete intake assessments for newly enrolled chronic care patients, educate them, coordinate resources, and keep care moving, all from home.

Schedule
- Full-time, 100% remote (US-based)
- Monday to Friday, no weekends
- Rotational on-call: about once per year on average
- Shifts: 8:00 AM – 4:30 PM CST OR 9:00 AM – 5:30 PM CST
- Most of your shift is on the phone
What You’ll Do
⦁ Accept warm transfers from the Patient Enrollment team and conduct preliminary health assessments for newly enrolled patients
⦁ Answer patient questions, create an open dialogue, and identify care needs early
⦁ Coordinate community resources that support chronic care management
⦁ Provide patient education and health literacy for chronic condition management
⦁ Support medication management by identifying concerns, adherence issues, and coordinating refills
⦁ Help ensure timely delivery of services like Home Health, DME, Home Infusion, and other critical needs
⦁ Take thorough, real-time notes during calls and document accurately
⦁ Adapt quickly in a fast-paced environment while maintaining strong attention to detail
What You Need
⦁ Active Compact/Multi-State LPN or LVN license
⦁ 3+ years of direct patient-facing experience
⦁ Strong written and verbal communication skills
⦁ Comfort working phone-forward all day in a high-volume environment
⦁ Technical aptitude with Microsoft Office Suite
⦁ Reliable high-speed Wi-Fi and a HIPAA-compliant home workspace

Pay
- Starts at $21/hr
- Can earn up to $28/hr based on production
